Alongside AdvisorsAdmission and Discharge Planning

The moves into treatment, between levels of care and home.

Programs are good at the weeks inside their walls. The risk sits at the edges: the decision to go, the day of discharge and the month that follows. This service is built for those edges.

What it includes

What we plan.

01/Choosing and entering a program

An independent look at the options, the admission paperwork and timing handled, travel arranged, and the family briefed. The choice always belongs to the client and the family.

02/Moves between levels of care

Residential to outpatient, outpatient to sober living, a transfer between programs. Each handoff planned, with the same Advisor present on both sides.

03/Discharge, planned before it happens

With the program's agreement and the client's consent, the Advisor meets the client while they are still in treatment, so the person waiting at discharge is already familiar.

04/The first thirty days home

First appointments booked before discharge and confirmed after. Housing settled. A written first-week calendar with at least one fixed commitment every day. The family briefed on what helps.

Who it is for

Who it is for.

  • Families deciding on treatment who want the logistics handled and an honest view of the options.
  • Clients approaching discharge whose aftercare recommendations need to become a schedule with names, dates and a ride.
  • Treatment programs and discharge planners who want the part of the plan that happens outside their walls to actually happen.

In the plan

How it fits the rest of the plan.

With the clinical team

The program sets the aftercare plan. We make it happen, report what we observe and refer back when something needs clinical attention.

With the family

One point of contact and regular updates within the limits the client sets, and a plan for the return home that everyone has seen.

What follows

A Personal Companion for the first days where the plan calls for it, and Ongoing Coaching as work, family and routine return.

Fees and independence

Private pay. We neither pay nor accept referral fees, and we do not steer anyone toward a particular center. Florida law requires it; we would do it anyway.

Good to know

Questions families ask.

Do you work with programs outside Florida and New York?

Yes. We work alongside the treatment team the client already has, in whatever state or country the program sits, and travel for admissions and discharges.

When should we call?

While the person is still in treatment, or while the family is deciding on treatment. That is when the plan for what comes after can still be shaped.

What is in the first-week calendar?

Appointments, meetings, meals, training, work or school, and the people responsible for each. At least one fixed commitment every day.
